Health services. This is a specialist falls prevention service which aims to reduce the number and impact of falls in older people.The service will be focussed on people 65 and over at risk of a fall, and will offer a one stop service providing:— patient education, information leaflets and lifestyle support,— advice and signposting for older people, their relatives, and health and social care staff,— multi-factorial assessments to people at high risk of falling,— multi-factorial interventions including individualised rehabilitation and exercise programmes for people who meet the service thresholds,— appropriate follow up to include onward referral to community geriatrician for medical assessment where appropriate,— work with carers/support networks to reduce risk and promote independence,— relevant awareness training and education to equip other health and social care workers across primary care and secondary care to enable them to recognise and reduce risk, and to access services on behalf of and for the benefit of patients.
