The CCG is leading an Integrated Care Programme (ICP) involving all local health and care organisations. The programme aims to improve outcomes for patients, create a more rewarding environment for staff and make better use of resources through greater integration. This includes population health management, MDTs working across organisational boundaries and helping citizens to take responsibility for their own health through a personalised and proactive approach. The business intelligence function is an integral element of this system ensuring that the ICP understands where the needs of patients are not being met and providing the right data evidence to support real transformational change in improving the quality of care.
