Descriptions
Young carers are defined as children and young people up to the age of 18 who have caring responsibilities for someone who has a physical or mental illness, a physical or learning disability or drug or alcohol problem.The service will provide an individually tailored package of support based on a statutory assessment of need and agreed by the young carer and their family. The service will strive to ensure that individual carers are helped to optimise their physical and emotional well being, supported to relieve the stress of the caring role, able to maintain social contacts and personal relationships, assisted to enjoy separate social and community activities and enabled to participate in leisure activities and education.The service will be available to young carers across Rhondda, Cynon, Taff aged 5-18.The period of the contract is for 3 years with an option to extend for 1 plus 1 years.Further information can be obtained by registering your interest in this notice and downloading the documents fromhttp://appswales.alito.co.uk/
