Lincolnshire CCG would like to invite providers to become a qualified provider on the AQP framework for Community Optometrist Triage, Assessment and Treatment Service (COTATs)., The service objective is to provide a high quality patient-centred Community Optometry service which raises the standard of care. The Commissioners will contract with provider(s) who will then carry out the service under this contract. The provider will deliver the following services to patients registered with a General Practitioner (GPs) within Lincolnshire., The Service will diagnose and treat a range of eye conditions within an overall model of care and manage relevant follow-up activity within the service before final discharge back to the care of the patient’s own Optometrist, GP or onward referral to either secondary care or a suitable community based specialist service such as the Community Surgery Scheme., Further information can be found via https://uk.eu-supply.com under Quote/tender reference 46214.
