South Devon and Torbay Clinical Commissioning Group's system-wide programme for prevention, wellbeing and self-care comprises of three interlinked delivery work streams:— Learning and development programme (workforce and citizen facing behavioural change programme);— Community asset development; and— Information assets.Via these three work streams we will establish the necessary self-care information resources and infrastructure, support the deployment of the learning and development at sufficient pace and scale, and support the development of resilience within communities we will require access to, and the development of, a range of specialist resources, products and additional capacity through 2017/18.
