This contract is to provide an integrated model of service to maximise ease of access to information, assessment, early intervention, equipment, rehab and support services for adults and children who are blind, partially sighted, D/deaf, hard of hearing or who have a dual sensory loss. This specification proposes to address this by seeking a single lead provider who can offer a range of seamless services via the network of sensory organisations.r r The Service will improve the quality of their life and should assist the service users to maximise their independence, help them maintain their health and promote and enhance effective personal support networks. The Service should support the service users to take greater control of their life and allow them to remain as independent as possible within their own home, their community and within their chosen way of life to reduce the needs of people over time.
