Descriptions
SNEE ICB, in collaboration with ESNEFT and local weight management providers and commissioners across the Integrated Care System (ICS), propose the creation of a Single Point of Access (SPoA) for all ICS weight management referrals. This is currently across the SNEE geography but is subject to change given future ICB changes, and therefore a scalable solution is needed.
This new innovation would serve to risk stratify and triage patients into the most appropriate initial weight management service pathway, navigating the patient in a timely manner and based on clinical need to either a local weight management provider, an NHS digital weight management offers (Digital Weight Management Programme, Diabetes Prevention Programme, Type 2 Diabetes to Remission programme) or the ESNEFT WMCOS which includes Community Outreach for GLP-1 medication delivery.
