The Community Ageing Well Service (CAWS), formerly Frailty Multi-Disciplinary Team (MDT), supports Barnet patients aged 65+. The NHS Core Frailty MDT Team is based at Edgware Community Hospital, CLCH. This contract is to fund the provision of a Frailty Multi-Disciplinary Team (MDT) Coordinator to work with the NHS, Age UK Barnet and other organisations, in support of NHSE's Ageing well/Living Well programme. The individual is non-clinical and employed by Age UK Barnet to support the Pan Barnet Frailty MDT by providing a wider and more holistic approach to patients referred into the MDT. The post holder will represent Age UK Barnet and the wider voluntary sector, taking referrals and identifying appropriate and holistic solutions for patients. The MDT Coordinator works alongside GPs and other health and care professionals to provide proactive support for older people in the Barnet community.
