The D2 Collaborative Children's home vision and approach: The aim is to deliver robust, needs-based provision that supports the health, therapeutic, care, and education needs of children and young people. This includes providing stable, intensive therapeutic support with trained care staff who follow consistent, multi-agency support plans. The approach looks to reduce fragmented care by establishing clearer joint processes and escalation pathways across Health and Social Care. In partnership with providers the service should be flexible and dynamic, offering wraparound support that continues as the child or young person transitions to their next home.
