The five CCGs in North Central London have committed to commissioning health services based on outcomes, with an initial focus on three population groups: 1.People with diabetes; 2.Older people with frailty 3.People with severe mental illness. This contract is for the finalisation, validation, piloting and implementation of survey instruments that will measure pre-specified patient level outcomes in the three value-based commissioning population groups in North Central London. The anticipated outcomes of the proposed service are as follows 1.Production of ready to use, locally validated patient survey instruments to measure the patient defined outcomes measures for the Value-Based Commissioning population groups: •People with severe mental illness •People with diabetes •Older people with frailty 2.Successful piloting of a methodology for delivery of survey instruments for each of the population groups.. 3.Completion of one full wave of questionnaires. 4.Provision of outcome data with 95% and 90% confidence intervals for each patient defined outcome measure for each population segment 5.Provision of data on response rate..
