Descriptions
Provision of a pharmaceutical and medicines support service to patients in their own home will include the following:— pharmaceutical assessments of person referred and reassessments and review by the Service Manager and designated assessors,— on-going support to eligible patients through the provision of an appropriate intervention by pharmacists,— administration including payment of participating pharmacists,— training, support and advice in medicine management to health and social care professionals and care providers.
