The Trust wishes to invest in a Trust-wide Patient Portal for patients under Acute and Community aspects of care. The Patient Portal will allow patients to: •Securely view their own healthcare record (including results) •Update appointments or their records •Access communication (e.g. Appointment letters, discharge summaries, Outpatient Outcome letters) and care plans. Currently the Trust provides no electronic means for patients to access their health record to check appointments, make adjustment requests for time/date changes for appointments or planned surgery dates.
