Descriptions
NHS Manchester CCG wishes to commission a pilot proactive Primary Care and Pharmacy/Medicines Optimisation Support Service for care home residents within the North Manchester locality; the service will provide care home residents with the appropriate level of support that they require. There are 33 registered care homes in North Manchester with a registered population capacity of up to 1 096 residents (figures correct as at June 2016). Within the encompassing service there are 2 lots: Lot 1 is the Primary Care offer to Care Homes and Lot 2 is the Pharmacy/Medicines Optimisation offer to Care Homes. Both of these lots make up the service. It is anticipated that this will be a project delivered by either one provider delivering both lots or two providers bidding independently to collaboratively deliver the service to support the care gap for residents within the care home population in North Manchester.
