Contract requirements for HCT for the provision of continence products:— Fully managed service by the contracted company for patients across Hertfordshire to include the data inputting of patient requirement forms and general call handling within agreed criteria as set by HCT and the Bladder & Bowel service.— Full range of continence products to be available including the provision of products from alternative suppliers should individual patients needs require this as determined by the Bladder & Bowel Service.— Delivery to the place of residence, continence products to those adults/children who have been assessed by HCT staff.— Regular deliveries to patients/residents as defined by HCT to be the most cost effective and efficient.— Patient activated deliveries (where appropriate) via a telephone system direct to the supplier to a dedicated customer services department.— Contract to include a second delivery attempt.— Costs should be provided for a contract WITH a collection of unopened surplus products from patient's homes and WITHOUT.— Home Delivery system to be managed via a state of the art database.— The system will be internet based and fully accessible to authorised users as agreed by the Bladder & Bowel Service.— To have a dedicated Contracts Manager for HCT.— Contract monitoring meetings to be held monthly.— Reports to be produced monthly and to be tailored to HCT requirements. Reports will be split across HCT Business Units and will include cost per patient per day; number of products per patient per day; type of products; delivery success rates etc.— Education to be provided by nurse advisors from the contracted company at no extra costs and in conjunction with the Bladder & Bowel Service.— Education and support will be ongoing to all HCT Business Units on the agreed product selection and will include all aspects of product choice; fitting; skin care etc.
