Descriptions
This Community Connectors service is commissioned jointly by NHS Gloucestershire Clinical Commissioning Group (GCCG) and Gloucestershire County Council (GCC). The aim of the service is to increase the wellbeing and resilience of individuals and communities and to reduce reliance on statutory services.This new service supports the Gloucestershire Health and Wellbeing Board's Enabling Active Communities policy, by building upon the work of the current Social Prescribing and Village and Community Agents and joining both provisions into one new integrated service.To access documents please register on our e-procurement portal: www.supplyingthesouthwest.org.ukPlease ensure that any questions and clarifications are sent through this e-procurement portal.
