The aim of the discharge to assess service is to provide an effective, flexible and responsive approach to discharge from the acute and surrounding hospital system for individuals who are medically stable and may require further assessment of their needs. This is part of the system response to the Covid-19 pandemic and seasonal pressures as well as supporting usual best practice.The joint service provision will provide discharge to assess capacity, within the Southampton and SW Hampshire locale, for individuals within Southampton University Hospital Foundation Trust who require a bed-based service but who no longer require consultant led care in an acute setting.
