Service to improve discharge arrangements for patients in hospital who need to be discharged with a package of care to support their rehabilitation. Service to provide additional community domiciliary capacity of 600 hours per week. The service aims to complement existing health and care discharge initiatives to ensure timely discharge from acute and community hospitals/services in order to maintain flow, by providing a short-term service with a reablement approach that will help people to return to their previous level of independence, maximising their health, safety and wellbeing. Patients will be discharged, assessed at home by the local authority urgent promoting independence (UPI) team who put the required level of care and reablement in place. The provider will support the UPI service to deliver this. At the point where it is determined that a package of care is required, the provider will provide a bridging service until the package of care is brokered.
