North East Hampshire and Farnham CCG intend to direct award to 4 existing integrated primary care providers operating within the geography of the Clinical Commissioning Group for Improved Access To Primary Care:— Farnham Integrated Care Services Limited (referred to as FICS within this document),— Oakley Health Group (referred to as Oakley Health within this document),— North Hampshire Urgent Care (referred to as NHUC within this document),— Salus Medical Services Limited (referred to as Salus within this document).The reasons for the proposal of this direct award are:— to provide consistency in delivery of a number of key locality based enhanced primary care services — recognising the positive impacts on patient outcomes and primary care provision that have been realised to date,— to recognise the contribution made by primary care super practices and federations in relation to the development of locality-based integrated care models throughout and now beyond the period of the “Happy, Healthy at Home” Vanguard Programme,— to recognise the vital importance of local clinical leadership in the development, mobilisation and success of enhanced primary care services,— and to support the continued essential role of enhanced primary care services within the out-of-hospital environment.The initial contract term will be 3 years with a possibility of a 2-year extension comprising of 1 year and 1 year. The period of time covered by this direct award is from 1.4.2019 to 31.3.2022 (to 2024 with the 2 extensions of 1 year each).
