NHS Guildford and Waverley Clinical Commissioning Group (the CCG) has redesigned the model of care for acute and community dermatology services. NHS Guildford and Waverley CCG serves a registered population of approximately 220 000 people across 21 GP practices.Historically, the CCG has commissioned acute and community services from 2 separate providers. There has been an overall increase in demand since the community service launched, and it is recognised that in order to manage future demand and deliver pathways in a seamless manner, a new integrated model of care is required.
