Descriptions
NHS Luton Clinical Commissioning Group and Luton Council are jointly commissioning an integrated health and wellbeing service that will work with our communities to proactively deliver a preventative approach that emphasises the whole person and which works to achieve the optimum levels of physical, mental, social and emotional health. The Luton wellbeing service will have one lead (Prime) provider who may work with local partner organisations under a single branding, promoting ‘one-service’, regardless of whether different organisations are delivering components of the service.The Luton Wellbeing service will be an integral part of Primary Care Home which is our Luton clinical model that enables the public to access wellbeing support in their locality.
