The Community Dermatology Service will provide high quality appropriate and prompt clinical care for a wide range of dermatology conditions in a community based setting. , The commissioned service will be expected to offer initial assessment, diagnosis and relevant treatment together with appropriate follow up care for dermatology patients being treated by the service, in line with NICE guidelines, for example the treatment of low-risk BCCs., Service provision will be for patients aged 18 years and over who are registered with a Mid-Hampshire GP., The provider will be responsible for the supply, safe installation, on-going maintenance and replacement as necessary of all suitable and relevant equipment and software required to provide the service. The service provider will be responsible for ensuring there is appropriately qualified staff necessary to deliver a fully accredited service, compliant with all appropriate national standards and regulations, including business continuity. , The provider will co-ordinate and manage the population need for Dermatology services, reducing dependence on referrals into secondary care; activity levels to be contained within agreed plans; covering provision of Advice and Guidance, Teledermatology, Face to Face and virtual modes for delivery of first and follow up appointments where clinically appropriate. They will develop strong local clinical relationships with both Primary and Secondary Care clinicians and work collaboratively to deliver and ensure patients receive an integrated, seamless pathway of care., The provider is responsible for the provision of the team(s) and administration support required to manage the service. , The service must accommodate all dermatology patients with the exception of: , •Patients under 18 years of age at time of referral, •Suspected Cancer 2WW referral , •Patients who require Phototherapy treatment, •Suspected connective tissue disorders, •Systemic illness with a rash, •Cutaneous vasculitis, •Immunosuppressed or have Gorlin's syndrome , •BCC Lesions that: , -Are on the nose and lips (including nasofacial sulci and nasolabial folds), or around the eyes (periorbital) or ears. , -Are greater than 2 cm in diameter below the clavicle or greater 1 cm in diameter above the clavicle unless they are superficial BCCs that can be managed non- surgically., -Are morphoeic, infiltrative or basosquamous in appearance. , -Have poorly defined margins or, •Are located: , -Over important underlying anatomical structures (for example, major vessels or nerves)., -In an area where primary surgical closure may be difficult (for example, digits or front of shin). , -In an area where excision may lead to poor cosmetic result., •If any of the above exclusion criteria apply, or there is any diagnostic doubt, the patient should be referred to the Local Service Multi-disciplinary Team., •The exceptions described above and high risk BCC's will be referred to the appropriate Secondary Care provider under the 2 week wait (urgent cancer referral guidelines)., •Conditions which are excluded from NHS funded treatments (See Schedule G), Full detail available within the Service Specification which can be accessed via the in-tend portal under project ref F15204 as per additional information section., It is anticipated that the legal landscape governing procurement of NHS contracts is due to change in 2022.
