Descriptions
NHS Scarborough & Ryedale CCG is looking to commission a multi-speciality community provider model for the delivery of a range of community based health and care services for its registered population. Whilst North Yorkshire County Council as a Commissioner of Services is not proposing to commission any social care services through this route at the outset it is fully committed to supporting the integrated care agenda and may, at the sole discretion of the Council, transfer a number of social care services and budget lines and functions to the chosen service provider during the life of the contract subject to Council approval and in accordance with the process set out in any resulting MCP contract.NHS England will be a co-commissioner in respect of any primary medical services delivered through this contractual route.It is anticipated that the provider will deliver some core services on from day 1 with the potential for additional services to be delivered from later points.
