Descriptions
The Community Anticoagulation service Pilot will be provided under a Community Integrated care model and will run for one year from 1st April 2016 to 31st March 2017 (with the option to extend for up to 1 year whilst evaluation and procurement takes place). There will be a prime provider managing the service with individual practices/ community providers and Pharmacies delivering the service as subcontractors or member practices. The prime provider will be the contract holder and will be responsible for ensuring contractual compliance, effective delivery of services and meeting Key performance Indicators (KPI’s).
