The Waltham Forest CCG is seeking to Procure a primary care-led service model for the monitoring and management of anticoagulation patients. The service is required to be delivered from a minimum of 5 sites across the borough, with at least 3 of these sites being local pharmacies. The proposed sites should provide sufficient coverage across the three localities in Waltham Forest: Leyton/Leytonstone, Walthamstow and Chingford.
