Continuation of an integrated contractual solution across local primary care provision comprising of: a Care Planning LIS delivered in primary care; delivery of a virtual ward and care navigation service, and enhanced Practice Nursing provided though General Practice. The overall service delivers multi-disciplinary and integrated anticipatory care in the community to support patients at high risk of hospital admission to prevent admission and provide care following hospital discharge. The Care Planning ensures GP led risk stratification and development of anticipatory care plans, subsequently managed by Enhance Practice Nurses. Care Coordination links GPs and Virtual Wards.
