Descriptions
Wiltshire CCG wishes to procure community-based Heart Failure Service for patients registered with a Wiltshire GP. The Community Heart Failure service will deliver the following:— rapid Access Diagnostic Service,— urgent Patient Review,— routine Patient Review,— post Discharge FU,— virtual ward management at home of unwell patients,— self-Management and education programme for HF patients The procurement is split in to 3 lots so as to provide maximum flexibility to both the Commissioner and bidders, and to ensure coverage of the full geography of Wiltshire. Contracts will be for an initial period of 3 years with a possible extension of any period up to a further 24 months, as defined at the discretion of the Commissioner. Wiltshire CCG has an annual budget available of 732 950 GBP with lot values based on the indicative volumes indicated in the tender documentation. This procurement is being carried out by NHS South, Central and West CSU on behalf of the Commissioners.
